Behavior Imaging’s Naturalistic Observation Diagnostic Assessment service is provided as a resource for diagnosing autism between the ages of 18 months and 7 years of age. The NODA service provided by Behavior Imaging requires family participation through the submission of video evidence. The video evidence is guided by clinical instruction capturing your child’s behaviors typically occurring in the home. The clinicians will observe all information provided by you to determine a diagnostic opinion based upon the Diagnostic and Statistical Manual of Mental Disorder criteria. The service contracts licensed and experienced clinicians with the Southwest Autism Research & Resource Center who are performing the diagnostic assessment.
Your participation in this service will be considered an indication of acceptance of:
• The diagnosis you will receive is based upon Behavior Imaging’s NODA method
• The NODA diagnosis is not intended to replace a full medical evaluation or early intervention plan that may be required to determine further appropriate treatment.
We strongly suggest that you carefully read the following terms and conditions. Because this service is accessible via our web site and iOS mobile application, you agree to abide by terms and conditions described below. These terms and conditions are subject to change at the discretion of Behavior Imaging.
Privacy Practices (Personally Provided Information)
If you choose to use the service, you will be providing us with personal information via the web site registration process by filling out forms and submitting it through our web site. You will also be providing video submissions of your child’s behaviors in various home settings that will be submitted to SARRC clinicians, and be used in a clinical diagnose. We use that information to respond to your request and to help us provide you with information, materials, and diagnostic information that are required to complete the diagnostic service. Behavior Imaging will hold your diagnostic information no less than 60 days from the time of final diagnosis. We do not give, share, sell, or transfer any personal information to a third party unless required by law or your personal written request.
Stored Information
We will learn certain information about you if you utilize the service. You should know that we will store the following information from your web use and video submissions of the mobile device. The information is used to measure the number of customers to the various sections of our site and identify system performance or problem areas. We also use this information to help us improve features of the technology and to make the site and mobile device more useful. All data collected on our web site is protected from unauthorized access. Personal information is encrypted using SSL (secure sockets layer) technology.
Site Access and Licenses
The NODA Service web site and mobile application access is permitted only for individual and private use by complying with the terms and conditions.
Credit Transactions
Behavior Imaging does not accept payment in the form of cash, check, money order, or insurance reimbursement. Major credit cards (MasterCard, Visa, American Express and Discover) are the accepted means of payment. Once submitted, Behavior Imaging maintains the right to utilize your credit card to make a onetime payment for the diagnosis rendered under the NODA service. You agree to allow us to charge your credit card for the service as it is rendered. In case your credit card expires or we cannot bill your credit card, we will send you a notice, and your request for service from Behavior Imaging will not be fulfilled until a single full payment has been received and/or verified. Upon receipt, please provide us with a valid credit card we can charge for the amount due.
State Jurisdiction
This Agreement shall be governed by and construed in accordance with the laws of the State of Idaho without regard to its conflicts of laws rules. Any dispute shall be brought in the courts of Idaho.
